Information Brochure for GPs

If you are interested in getting a Mental Health Treatment Plan (MHTP) from your GP I would encourage you to email me: felicity@mothersbeheard.com  so I can send you a PDF version of below.  It is hard to read in this website format and having a printable copy would mean that you could more easily show your GP when you next see them. 

 

Getting a MHTP is easy to do and, remember, there is nothing to be ashamed of if you are not feeling your normal self.  Also you do not need to have a clinical diagnosis of Postnatal depression to get a MHTP. 

 

Your GP will need my fax number - (08) 7200 3268 - so they can send your referral through to me.  I will then be in contact with you to work out the best time to see you.  I work Wednesdays and Thursdays.

 

Please remember that if you do email me to get a better version of my brochure, I will respect your privacy and I will not place any pressure on you at all to follow up with this service.
